Request an Absentee Ballot

- By mail - Complete the Absentee Ballot Request form (PDF) and provide a copy of your Proof of Identification (PDF) to Village Clerk's Office at W156N8480 Pilgrim Road, Menomonee Falls, WI. The deadline to apply by mail is 5 p.m. on the Thursday before the election.
- In-person - During open office hours, the two weeks prior to an election, voters may come to the Village Clerk's Office to apply for an absentee ballot to be sent to them. You will need to still fill out the Absentee Ballot Request form and show proof of identification.
In-Person Absentee Voting
In-person absentee voting takes place two weeks, starting with Tuesday, before each election and ends at 5 p.m. on the Friday preceding the election. Voters may come into Village Hall, show proof of identification and receive an absentee ballot. Remember you must be registered to vote before requesting an absentee ballot. Please view our election notices as to specific dates and times of in-person absentee voting.

Deadline to return an absentee ballot
Absentee ballot must be received by the municipal clerk on or before election day. If the absentee ballot is received by the municipal clerk on election day, it must be received in time for the clerk to deliver the absentee ballot to the polling place before 8 p.m.
