The Voter Registration Form (GAB-131) must be completed accurately and completely. First-time Wisconsin voters who register by mail need to provide proof of residence. The forms should be mailed to: Village Clerk's Office, W156N8480 Pilgrim Road, Menomonee Falls, WI 53051. You may also register at one of the following locations:
- The Village Clerk's Office
- Menomonee Falls Public Library
- The Polls on Election Day
- At myvote.wi.gov
Be aware that there may be time restrictions that registration by mail is accepted. The application must be postmarked not later than the 20th day (third Wednesday) before an election.
If you are not already registered, you will need to register to vote before an absentee ballot can be sent to you. All requests for an absentee ballot must be in writing by the elector and need to include proof of identification. A photo ID is now required to receive a ballot in all special and regular elections.
Request by Mail
Complete the Application for Absentee Ballot form and mail it to the Village Clerk's Office at W156N8480 Pilgrim Road, Menomonee Falls, WI 53051. The application must be received by the clerk no later than 5:00 PM on the Thursday before the election in order for an absentee ballot to be mailed to you.
Request in Person
You can vote an absentee ballot in person at the Village Clerk’s Office. In-person absentee voting begins on the 3rd Monday before Election Day and ends at 5:00 PM on the Friday preceding the election.
Deadline for Returning Ballot
The completed ballot must be received by the Village Clerk’s Office no later than 5:00 PM on the day of the election so that it can be delivered to the polling location by 8:00 PM. If postmarked by election day, the ballot may be eligible to be counted after election day.
